Parental Agreement for school to administer medicine
If your child needs medication, please see Mrs Riley in the school office. You will be asked to complete a form giving parental consent and also to specify the dose and time that the medicine should be given. Mrs RIley is responsible for all medicine administration and record keeping and she will contact you if there is a problem.
